First Visit
You should arrive 15 to 20 minutes early to allow enough time for new patient forms to be completed. Alternatively, you can download these from our website or have them
faxed to you at an earlier date.
Your first visit will consist of reviewing your medical history, physical exam by the doctor and treatment as needed. At this visit you will be detailed on further testing and treatment plan.
Things to bring with you for your visit are:

Your current insurance card

Complete list of all prescriptions and any over-the-counter medications, including the dose frequency.

Physician referral forms, if they are required by insurance.

Information about your medical and surgical history.

Recent x-rays, CT scans or relevant medical records as instructed by our office or by your primary care doctor.
Subsequent Visits
Please notify the office if there are any changes in your name, billing address, telephone number or insurance carrier. Please remember that you are an active participant in your medical care. It is advised that you write down your questions in advance and always bring in your medication lists for us to review at each and every office visit. This is also the best time to refill medications.
